Signed A&E Application: Must be fully completed and signed by a firm principal.
5-Year Loss Runs: Currently valued within the last 90 days.
Claims Circumstance Report: A detailed narrative for any losses recorded in the last five years.
Project List: A summary of the largest active projects by billings or construction value.
Current Policy Documentation: Including Declarations pages and any specialized endorsements.
